    • You may edit this document on your computer.

    • Please provide completed type-written pages

    • Please return the completed pages in word document format via e-mail

  • Digital pictures of your loved one’s life. 40 Maximum.

When determining the pictures to send: 

  • As your loved one ages and memory skills change, they recognize themselves as younger people vs. the older person they are now.

  • Pictures from their youth, when they were a young spouse or parent, will be most comforting.

 

Please send a cross-section of pictures:

  • From toddler to youth

    • Your loved one with brothers and sisters, graduation/high school, and college

  • ​​​​Adult:

    • Wedding picture

    • With children and grandchildren

 

For the Highest Quality Pictures: Please provide digital images at a high resolution. The photos must be in jpeg or png format.

 

With each set of pictures:

  •  Include :

    • Names of those present

    • The Event/Occasion

    • The date if known

Example Caption: 1. 1911: Blanche’s mother, Lizzie, Norma’s mother, Blanche, Blanche’s brother Leslie, on the homestead in Minnesota.

Interesting Personal Facts: Within the personal History and Abilities Pages, you will find Interesting Personal Facts. If you choose to complete this page, consider this example: Mom is one child in a set of identical twins. She met President Johnson in 1968.

 

Please Note:

Should you choose this purchase option, your time investment would be 2-4 hours.

Which would include:

  • Choosing your photos and labeling them 

  • Completing the Personal History and Abilities Pages, Interesting Personal Facts Page 

  • Uploading the information to the website

  • Attending An Engagement Meeting

 

Some suggestions for the cover and the title page picture:

 

If the Subject is:

 

  • An aging family member who is experiencing changes in memory and communication skills consider:

    • As your loved one ages and memory skills change, they recognize themselves as younger people vs. the older person they are now.

      • Therefore, consider a cover photo at a young age (a glamour shot) and an interior title page photo at an older or current age.

  • An active senior citizen, consider a cover and title page photo as the older adult they are now.

  • An adult 20-50 years-old (or younger) with special needs, consider a cover and title photo at current or more youthful age.

It is all up to you! This is your book created for you, your family, and your caregivers. We want to make it your way.

In the interest of space and formatting concerns, the author reserves the right to format/arrange photographs and provide appropriate captions as is necessary to provide the highest quality product.

 

Upon completion, before publication, we will e-mail a PDF proof copy for your review. Please review the information carefully. If changes need to be made, the first five  (5) individual changes (picture and or text) are free of charge. Any changes beyond five (5) will incur a $5.00 per change charge.
